DSCESU Forms
** Sponsors and PIs - please refer to the funding agency's cooperative agreement number and the DSCESU in your proposal and task agreement to ensure you receive the reduced indirect rate. The cooperative agreement number is in the upper right corner on the first page of the Cooperative & Joint Venture Agreement. **
A DSCESU Project Summary Form should be submitted to the DSCESU Program Coordinator for all project proposals funded through the DSCESU Cooperative Agreement.
National Park Service required forms (FY 12)
Complete and submit these six (6) items:
2) Scope of work / Project Proposal
3) Substantial Involvement Documentation Task Agreement
4) SF-424 - Application for Federal Assistance (signed by your institution's signatory - at the UA this is signed by the Office of Sponsored Projects)
For natural resource projects, submit to Larry Norris, NPS Southwest Research Coordinator, at lnorris@ag.arizona.edu.
For cultural resource projects, submit to Pat O'Brien, Cultural Resource Specialist, at Pat_O'Brien@nps.gov.

Work with your department's business office to submit the following documents to the Office of Sponsored Projects (520-626-6000, sponsor@email.arizona.edu):
F&A rate is 17.5%. In the box for justification of the reduced rate, write in the funding agency's cooperative agreement number, found on page 1 of the Cooperative and Joint Venture Agreement.
2) Project abstract
3) Project budget
4) A copy of p.17 of the Cooperative and Joint Venture Agreement
5) Completed 424 forms.
